    VA Aid & Attendance: How It Pays for Home Care

    Most families never hear about this benefit until someone happens to mention it. If you or your spouse served during a wartime period and now need help with daily activities, there's a real chance the VA will pay a meaningful part of your home care costs — tax-free, with no repayment required. This is the benefit our own family has navigated personally, not just a program we read about.

    What VA Aid & Attendance Actually Is

    Aid & Attendance is not a disability benefit, and you don't need a service-connected disability rating to qualify for it. It's an enhanced VA pension — an add-on to the basic Veterans Pension — designed specifically for wartime veterans and surviving spouses who need help with things like bathing, dressing, meal prep, or supervision due to a cognitive condition like dementia.

    The money is paid directly to the veteran or surviving spouse, and it can be used to pay for care at home, not just in a facility. That last part surprises a lot of families — many assume VA long-term care benefits only apply to nursing homes or VA facilities. They don't.

    Who Qualifies

    Three things generally have to be true:

    • Wartime service. At least one day of active duty during a VA-recognized wartime period (or the surviving spouse of a veteran who served).
    • A documented care need. Help with daily activities, being substantially confined to home, or a cognitive condition requiring supervision.
    • Financial eligibility. A net worth limit that adjusts annually, along with an income test — though unreimbursed medical and care expenses can reduce countable income and increase what you actually receive.

    As of the current VA pension rate table (effective December 1, 2025), the maximum monthly amounts are roughly $2,874 for a veteran with a spouse or dependent, $2,424 for a single veteran, and $1,558 for a surviving spouse — though your actual payment depends on your income after deductible medical expenses. These figures adjust with the annual cost-of-living increase, so we'd always point you to VA.gov's current pension rate table rather than treat any number as fixed.

    Applying for Aid & Attendance

    The application itself (VA Form 21P-527EZ for veterans, or 21P-534EZ for surviving spouses) can be filed online, by mail, or in person at a regional VA office, and processing can take anywhere from a few months to over a year. Benefits are generally paid back to the date you filed, not the date of approval, so filing as soon as you believe you're eligible matters more than waiting until everything feels certain.
