What Is a Geriatric Care Manager?

What a Geriatric Care Manager Actually Does
A geriatric care manager — sometimes called an Aging Life Care Professional — is typically a nurse or social worker who specializes in coordinating care for older adults. They assess a person's overall situation, help families understand options, coordinate between doctors and other providers, and often step in during a crisis (a hospitalization, a sudden decline) to help a family make fast, informed decisions without duplicating effort or missing something important.
What They Don't Do
A geriatric care manager doesn't provide hands-on daily care themselves — they're not going to help with bathing, dressing, or meal prep. That's the role of a home care agency or family caregiver. Think of a geriatric care manager as the strategist coordinating the plan, and a home care agency like ours as part of executing it.

This role tends to matter most when a family is geographically distant from an aging parent and can't manage day-to-day coordination themselves, when a situation is genuinely complex (multiple medical conditions, complicated insurance or benefits questions), or during a crisis moment when decisions need to happen quickly and nobody in the family has the bandwidth or expertise to sort through options calmly.
