VA Form 21-2680, Examination for Housebound Status or Permanent Need for Regular Aid and Attendance, is completed by your loved one's physician. It documents the specific daily activities they need help with — not a vague statement of poor health. This is the single most important supporting document, and vague or incomplete documentation here is one of the more common reasons claims stall.

Supporting Documents to Gather

• Discharge papers (DD-214) showing wartime service dates
• Marriage certificate, if applying with or as a dependent spouse
• Financial documentation: income, assets, and unreimbursed medical expenses
• The completed VA Form 21-2680 from a physician
