VA Home-Based Primary Care: A Medical Team, Not Just an Aide

This is the most medically intensive of the VA home programs we've written about — worth understanding clearly, including why it's different from what we provide.

What HBPC Actually Is

Home-Based Primary Care (HBPC) sends a full VA health care team — led by a VA doctor, and typically including nurses, therapists, and other clinical staff — directly into a veteran's home to manage complex, chronic medical conditions. It's built for veterans whose medical needs make regular clinic visits difficult, not for veterans who mainly need help with daily activities like bathing or dressing.

Who Qualifies

HBPC is only available to veterans already enrolled in VA health care, and specifically for those with complex chronic conditions who'd benefit from ongoing medical management at home rather than periodic clinic visits. VA is actively expanding this program — adding home-based primary care teams to areas with the highest unmet need as part of a broader 2026 push to help veterans age in place.

What the Team Actually Does

Nursing care, physical therapy, and medical management happen in the home rather than requiring travel to a VA facility. This is fundamentally different from the Homemaker/Home Health Aide program we've also written about — HBPC is skilled medical care led by a doctor, while H/HHA is non-medical personal care and homemaking support. A veteran can potentially use both, since they serve different needs.

Where Interim Fits — and Doesn't

We're a non-medical home care agency. We don't provide the kind of skilled medical management HBPC delivers, and we wouldn't want to suggest otherwise. What we do provide is the non-medical support that fills the rest of the week around a clinical team's visits — personal care, safety, companionship, and the daily routine support that HBPC isn't designed to cover. If your loved one has an HBPC team involved, we coordinate around their plan rather than working independently of it.
